Who should take the questionnaire?
You should take this questionnaire for yourself or for anyone else whose health or medication you manage, such as your children or family members, to help facilitate productive conversations with your providers.
How do I share this information with my provider?
You have the option to save or share the report, or simply show the report to your provider from your mobile phone! Your provider can use this information to help you during your next consultation.
Will any of my information be shared or saved?
You have the option to add your report to your saved questionnaires list in the app. You also have the option to email your report, or add your report to your preferred cloud storage. Note: it is not advisable to transmit personal health information. We do not share your confidential information.
